Palliative Care    

The Palliative Care program extends the principles of hospice care to a broader population of people who could benefit from this type of care earlier in their illness. The program is designed for people with a serious illness, regardless of life expectancy. Our Transitions program focuses on improving the quality of a seriously ill person's life and support that person and his or her family. The program is designed to help people by:

  • Relieving pain and other physical symptoms
  • Coordinating care between health care providers
  • Assisting patients and families in making informed choices about their care and treatment
  • Providing resources to address practical and financial needs.

The program has a nationally recognized “virtually integrated” program with the Grand Junction VA Medical Center, featuring expert consultations from physicians board certified in palliative care, and a care coordination program.

The Services Include:

  • Consultations by specialized doctors and nurses at home, nursing homes and assisted living centers
  • Care Coordination program that includes nursing visits, personal care, telephone support and volunteers
  • Individual and family support offered through counseling, support groups and education.
